This 2500-word assignment requires students to complete a patchwork report, with each section attributed to a module learning outcome. The patchwork order is outlined below. Please stick to this outline. The learning outcomes attributed to each patch need to be achieved to pass the assignment, which will be fine graded.
You will need to provide a reference list at the end of each patch. There is no set number of references required for each patch, this will be determined by the quality and depth of student writing.
This assignment can be written in the first-person narrative.
The learning outcome for the modules are stated here and provided the structure for the patchwork report.
Learning Outcome 1: Critically evaluate the mechanisms that can be used to influence organisational change and public policy, demonstrating the development of political awareness and skills.
Learning Outcome 2: Critically evaluate the service user experience based on evidence, audit and evaluation, to establish potential for improvement in a defined aspect of care/professional practice.
Learning Outcome 3: Demonstrate an ability to lead and co-ordinate personalised care and social prescribing across integrated care services, recognising the different levels of seniority and experience required to enable effective escalation through prioritisation and delegation.
Learning Outcome 4: Critically reflect on own leadership style and the impact of this in clinical practice on team performance and patient experience.

Patch 1 (LO2):
Instruction:
Identify an area of health care in the UK related to your field that uses an integrated approach. Using supporting evidence, critically discuss how an integrated approach has improved outcomes or professional standards in this area. (550 words)
Here are some examples of health care that uses an integrated approach. You can use these examples if you wish, although students choose their own.
• Adult: Diabetes care in the NHS
• Mental Health: Early Intervention in Psychosis services (EIP)
• Child: Children’s Palliative & Complex Care
Further Guidance:
· Avoid being too broad choose a focused clinical pathway or area of health.
· Remember the difference between multidisciplinary and integrated.
· You do not need to pick an area of integrated health you have experience in it can be an area you have researched relating to your field.
Patch 2 (LO3):
Instruction:
Identify an example of social prescribing that aligns or links to the area of healthcare identified in patch 1 and then demonstrate how and why personalised care should be upheld and consider escalation when applying social prescribing in that context.

Guidance:
· Hypothetical scenarios or simulated scenarios can be used for this as students may not have had any prior social prescribing experience.
· ‘Escalation’ in this context means being aware of what steps you would take if social prescribing activities were no longer suitable or appropriate for the level of care required for an individual. How would you monitor this and what steps you would take as the nurse overseeing this social prescribing.
· Dual students can pick one example from either field being studied.
Patch 3 (LO1):
Instruction:
Examine the tension between public policy objectives for integrated care and the operational realities of the UK health and social care sectors. Drawing on public policy since 2022 (listed below), identify the primary challenges to integration and justify potential solutions for achieving a ‘joined up’ system.

Guidance:
· The public policy you may want to choose from: Health and Social Care Act (2022), The 10 Year Health Plan (2025), Better Care Fund Framework (2026/27), The Model ICB Blueprint (2025).
· Search for summaries of what these pieces of integration policy are trying to do. Then consider the challenges from the point of view of a hospital, or healthcare service that is being asked to make these changes.
· Identify no more than two challenges to keep within the word count.
Patch 4 (LO4):
Instruction:
Drawing on a positive nursing leadership experience observed in either practice, simulation, or previous employment, critically reflect on the specific leadership competencies you witnessed in action. Analyse why these competencies were effective in that context, consider any limitations or challenges associated with them, and provide a clear, evidence-based rationale for how and why you intend to adopt these competencies to support integrated care delivery in your future nursing career.

· Remember leadership happens every day in healthcare from senior management down to support staff, it can be the small acts of leadership that can just as significant.
