Instruction:

Identify an example of social prescribing that aligns or links to the area of healthcare identified in patch 1 and then demonstrate how and why personalised care should be upheld and consider escalation when applying social prescribing in that context.

·       Hypothetical scenarios or simulated scenarios can be used for this as students may not have had any prior social prescribing experience.

·       ‘Escalation’ in this context means being aware of what steps you would take if social prescribing activities were no longer suitable or appropriate for the level of care required for an individual. Drawing on public policy since 2022 (listed below), identify the primary challenges to integration and justify potential solutions for achieving a ‘joined up’ system.
